When you click the button below you will be redirected to another page to complete the registration form.

We will ask you for some basic information about yourself such as name and contact details and which group dates would suit you. We will ask you the name of the person who died, and your relationship to them, so you are not placed in a group with someone else who knew them.   

Finally we ask you some questions about how you are feeling now.  We will ask the same questions again at the end of your group in order to understand what impact the groups have and to learn from your comments so we can improve the service for future groups.
